Ready to make Remedy your pharmacy? Here is how to make the move. We are so happy to have you!
New prescription from your doctor: Just ask your doctor’s office to send your prescriptions to Remedy Pharmacy.
Current prescriptions from another pharmacy: We can call your old pharmacy for you to get them moved to Remedy. Please call us and let us know what prescriptions you need and what pharmacy we should call.
Set up your patient profile. Due to HIPAA rules this must be done in store. However, if you would like to save some time and complete the form beforehand, feel free to download our new patient form and bring it with you.
Information needed to set up your profile:
Basic contact information (have your driver’s license handy)
Health information (your doctor’s name, your health conditions, allergies and current medications)
Your insurance information (if applicable). PLEASE make sure to have your insurance cards with you. Sometimes your prescription insurance information will be on a separate card than your medical insurance information. Please bring the card that has “RXBIN” mentioned somewhere on the card.
